How to File a Long Term Disability Claim
If you will be off work longer than your Long Term Disability waiting period, due to your own medical condition, you may be eligible to receive a benefit from Standard Insurance. WSU cannot ensure that you would be eligible for a benefit, because the insurance company will be responsible for reviewing your request. However, you are encouraged to complete the forms, since you may be eligible to receive a benefit.
To submit a claim you should contact either WSU Human Resource Services or Standard Insurance and request a Long Term Disability claim packet (policy number 377661-B). The claim packet is a three part process that includes:
- Employee Statement - to be completed by the employee
- Health Care Provider Statement - to be completed by the employee's health care provider
- Employer Statement - to be completed by WSU Human Resource Services
All forms should be completed and submitted to WSU Human Resource Services. Upon receipt WSU Human Resource Services will complete the Employer's Statement and forward it to Standard Insurance.

Long Term Disability Plan Booklet
The Genetic Information Nondiscrimination Act of 2008 (GINA) prohibits employers and other entities covered by GINA Title II from requesting or requiring genetic information of an individual or family member of the individual. To comply with this law, we are asking that employees and departments do not communicate any genetic information or medical information at the department level. “Genetic information” as defined by GINA includes an individual’s family medical history, the results of an individual’s or family member’s genetic tests, the fact that an individual or an individual’s family member sought or received genetic services, and genetic information of a fetus carried by an individual or an individual’s family member or an embryo lawfully held by an individual or family member receiving assistive reproductive services.
